Admission documents

The following documents will be required (for all admissions):

  1. Patient as well as main medical aid member’s identity document / passport / valid driver’s license.
  2. Latest medical card or medical aid certificate.
  3. Doctor’s note with relevant procedure codes.
  4. Medical aid authorisation number.
  5. COVID-19 vaccination certificate.
  6. Any additional documents required by your medical aid/type of admission e.g. workman’s compensation act.
